Health care and services for seniors are seriously fragmented in our system. Our project is seeking to create a learning clinic unit for seniors’ health to put an end to silos and ensure that stakeholders are aligned in matters pertaining to seniors’ health. The unit consists of a community of patients, doctors, health care professionals, decision makers, researchers and relevant partners. This group identifies and prioritizes major problems in the continuum of seniors’ care and works together to identify solutions based on research and evidence data.
We have consulted with patients, senior management, medical and clinical leaders and health care teams of the Vitalité Health Network. The services and health care provided are focused on patients to better meet the acute needs of an aging population. There is a huge sense of urgency with respect to the system’s ability to address challenges confronting seniors. It has also been noted that current health care and services are not adapted to seniors. We have also identified seniors as a strategic priority for our organization.
Our project will be offering new initiatives focused on senior patients. For example, these initiatives will deal with needs relating to health care, services provided, accessibility to services, etc.

Il existe une importante fragmentation des services et des soins de santé aux aînés dans notre système. Notre projet vise la création d’une Unité clinique apprenante sur la Santé des aînés afin de briser les silots et assurer un alignement des parties prenantes en matière de santé des aînés. L’unité est composée d’une communauté de patients, de médecins, de professionnels de la santé, de décideurs, de chercheurs et de partenaires pertinents. Ce groupe identifie et priorise des problèmes importants dans le continuum de soins des personnes aînés et travail ensemble pour identifier des solutions qui sont informées par la recherche et les données probantes.
Nous avons consulté des patients, la haute administration, les leaders médicaux et cliniques et les équipes soignantes du Réseau de santé Vitalité. L’offre de services et soins de santé centrés sur le patient afin de mieux répondre aux besoins criant de la population veillissante. Il existe un grand sentiment d’urgence par rapport aux capacités du système d’addresser les défis en matière de santé pour les aînés. On note aussi que les services et soins de santé actuels ne sont pas adaptés aux aînés. Nous avons aussi identifié la santé des aînés comme une priorité stratégique pour notre organisation.
Notre projet apporte de nouvelles initiatives axés sur le patient aîné. Ces initiatives tiennent, par exemple, des besoins en termes de soins de santé, de services offerts, d’accessibilité aux services, etc.
