What to bring to your appointment

Bring as much documentation and information about your income and potential deductions as possible. We will also need documentation of your spouse/common-law partner’s income and any dependents.

Useful documentation includes:

  • Identification (social insurance card, driver’s license, or Medicare).
  • Proof of income (pay stubs received in the last thirty days prior to the date of application, income assistance stubs or Employment Insurance statements; other proof as required).
    • Seasonal employees: Bring your most recent Income Tax Notice of Assessment in addition to the proof of income as indicated above.
    • Social assistance benefits: If you are receiving, please bring your most recent deposit slip and your white or yellow health card issues by Social Assistance.
  • Proof of allowable deductions (receipts).
  • Court documentation – bring any existing documents related to your case (court orders, separation agreements, etc).
