The interRAI Long-Term Care Facilities (interRAI LTCF) © is part of interRAI’s new integrated suite of assessments that use common assessment items and standardized language to support continuity of care across health care settings. Based on rigorous research, these new assessments are being adopted to flag health risks and to guide comprehensive, person-centred care planning and services.
Like other interRAI tools, the LTCF is a comprehensive, standardized system for evaluating the needs, strengths, and preferences of persons in chronic care and nursing home settings. Completing an LTCF assessment enables a health care provider to assess key domains of function, mental and physical health, social support, and service use.
The LTCF is an assessment tool used by trained healthcare professionals in New Brunswick nursing homes to collect information upon a resident’s admission and repeated quarterly thereafter, and with any significant change, to inform the resident’s clinical care plan.
