The information collected on this application form will be used solely for the purposes of assisting to assess my participation as a student-athlete. The information will be accessible to coaching staff, UNB Saint John Athletics, UNB Recruitment, and UNB Associate Registrar.

As a high school student, I acknowledge that my parent or guardian will be contacted first to obtain permission for my participation.

No use of personal information about me beyond the terms described above is permitted without my express written authorization or unless permitted by law.

This agreement does not include, or supersede other personal information agreements signed at the University of New Brunswick concerning my personal information.

This consent to use my personal information expires two months from the date of submittal.
