Submitting this Equipment Booking Request Form will send an email with the specifics of your request to the CETL Equipment Pool.  You may also contact the Equipment Pool directly at 458-7662.

PLEASE NOTE: This is a request only.  Confirmation from our Equipment Pool staff is required before you may consider that any equipment reservation or delivery request is actually scheduled.

• Users are responsible for care and prompt return of equipment.
• Customers will be subject to fines for overdue or damaged equipment.
• Rental rates and charges may be viewed at Equipment List.
